Our New York region was established in 2005 with the opening of our Westchester Wraparound Program. NAFI built upon the success of this initial program in the following years and we now have multiple community-based service offerings throughout the state. We are best known for providing intensive, community based wraparound services as alternatives and transition from residential placements that include foster care, preventive service, aftercare, consultation and individualized support services. Our services support youth with complex needs, gender specific services for girls and their families, relative caregivers, foster families and birth families in developing stability and growth.

WRAPAROUND SERVICES

NAFI NY incorporates elements of the Wraparound Approach 1) Family voice and choice: family and youth/child perspectives are central during all phases of the wraparound process. 2) Team based: the wraparound team consists of individuals agreed upon by the youth and family and committed to them through informal and formal supports, and community support services (mentors, boys and girls clubs, YMCA’s, parks and recreation, youth vocational training programs, art’s programs etc.). 3) Natural supports: the team encourages full participation of members drawn from family members networks of interpersonal and community relationships. 4) Collaboration: team members work cooperatively and share responsibility for developing, implementing, monitoring, and evaluating a single wraparound plan. 5) Community-based: the team implements service and support strategies that take place in the most inclusive, most responsive, most accessible, and least restrictive settings possible; and that safely promote child and family integration into home and community life. 6) Culturally competent: the wraparound process demonstrates respect for and builds on the values, preferences, beliefs, culture, and identity of the child/youth and family, and their community. 7) Individualized: to achieve the goals laid out in the wraparound plan, the team develops and implements a customized set of strategies, supports, and services. 8) Strengths based: the wraparound process and the wraparound plan identify, build on, and enhance the capabilities, knowledge, skills, and assets of the child and family, their community, and other team members. 9) Continuation of Care: Services and supports must be provided unconditionally. In a crisis, services and supports should be added rather than placing the youth with a new provider. 10) Flexible Funding: Flexible Funding and Resources: flexible funds and resources to implement their ideas. 11) Outcome based: the team ties the goals and strategies of the wraparound plan to observable or measurable indicators of success.

With consistent growth within the organization, NAFI NY is balancing expansion with a focus on continued quality care. Best practice research and program data collection is applied to develop systems that ensure adherence to quality improvement measures that are woven through and across NAFI NY.
